Janian Medical Care

Janian Medical Care is our award-winning health care program. Established in 2011, Janian delivers a matrix of psychiatric and primary medical care services to homeless and formerly homeless individuals throughout New York City.

Named in the spirit of Janus, the Roman god of transition, change, and new beginnings, Janian is on the forefront of providing compassionate, seamless care to people with complex medical and mental health illnesses.

Psychiatry

As the largest provider of psychiatric care to homeless and formerly homeless people in New York City, our Janian psychiatrists are the vanguard of helping people with serious mental health illnesses.  

On the streets, in soup kitchens, shelters, housing programs, and in communities throughout the city, we provide person-centered, recovery-oriented psychiatric care that integrates mental health, substance use, and physical health to the most vulnerable New Yorkers.

Primary Medical Care

Our primary medical care program provides personalized health care on-site at shelters and supportive housing programs.

Breaking the mold of fast-paced health care, the program has created a new paradigm for care, treating patients where they are, building trust with the same healthcare provider and ensuring that patients fully understand their conditions.  

Street Medicine Program

The first of its kind in New York City, our street medicine program brings medical care to park benches, underpasses and anywhere needed with the purpose of helping homeless people address their health problems.

Designed and licensed to deliver medical care wherever homeless people are, our medical practitioners travel freely to locate, engage and treat patients. Our street medicine practitioners are the vanguard of care for those disconnected from the traditional health care system and most in need.

  • Van Yu, M.D.

    Dr. Yu earned his M.D. at the State University of New York at Stony Brook and completed residency training at New York University Medical Center where he was a Chief Resident.  Prior to joining Project for Psychiatric Outreach to the Homeless (PPOH) in the spring of 2003 he was an Attending Psychiatrist and then Assistant Director of the Bellevue Hospital Center Comprehensive Psychiatric Emergency Program.  Dr. Yu is a Clinical Assistant Professor at the State University of New York Downstate Medical Center and at New York University Medical Center.

    Dr. Carino earned his M.D. at the Loyola University Stritch School of Medicine and completed residency training at the Albert Einstein College of Medicine, Montefiore Medical Center where he was a Chief Resident.  He completed fellowship training in Community Psychiatry at the New York State Psychiatric Institute, Columbia University Public Psychiatry Fellowship.  Prior to joining PPOH in July 2010, Dr. Carino was the Associate Medical Director at Pathways to Housing in New York City.  He is an Assistant Professor of Clinical Psychiatry at the College of Physicians and Surgeons at Columbia University and is on faculty at the Columbia University Public Psychiatry Fellowship. He is on the Board of Directors for the American Association for Community Psychiatrists. Dr. Carino was a 2013 recipient of the APA Nancy C.A. Roeske, M.D., Certificate of Recognition for Excellence in Medical Student Education.

    Dr. Fried earned her M.D. at the Columbia University College of Physicians and Surgeons and completed residency training at New York University Medical Center where she was a Chief Resident.  She completed fellowship training in Community Psychiatry at the New York State Psychiatric Institute, Columbia University Public Psychiatry Fellowship doing her clinical placement with PPOH before joining PPOH as a staff psychiatrist in July 2010.  As Assistant Medical Director for PPOH, Dr. Fried also serves as Medical Director for the Manhattan Outreach Consortium.  Dr. Fried is a Clinical Instructor at New York University Medical Center.

    Dr. Maddox earned her M.D. at the State University of New York at Stony Brook and completed residency training at the Albert Einstein College of Medicine, Montefiore Medical Center where she was a Chief Resident. She completed fellowship training in Community Psychiatry at the New York State Psychiatric Institute, Columbia University Public Psychiatry Fellowship doing her clinical placement with PPOH before joining PPOH as a staff psychiatrist in July 2007. Dr. Maddox left PPOH in 2011 to become the Director of the Mobile Crisis Team of the Kings County Medical Center Comprehensive Psychiatric Emergency Program. Dr. Maddox returned to Janian Medical Care in the spring of 2014.

    Dr. Calhoun earned her M.D. at the Columbia University Ben-Gurion University Medical School in Be’er Sheva, Israel. She completed residency training at the State University of New York Downstate Medical Center and at Maimonides Medical Center in Brooklyn, New York where she participated in research on Buprenorphine. She also completed a Public Psychiatry Fellowship at the Nassau University Medical Center. Dr. Calhoun joined Janian in 2015.

    Dr. Colton earned her M.D. from Yale School of Medicine, and she completed her psychiatry residency training at Weill Cornell Medical/New York Presbyterian Hospital. She joined PPOH in 2011. She took on the role of Assistant Medical Director of Psychiatry in 2022.

    Ms. Gerteis earned her Masters of Science in Nursing in Family Practice at the Columbia University School of Nursing in 2014.  She joined Janian as a Primary Care Provider in 2014, became Janian’s first Nursing Supervisor in 2016, became an Assistant Medical Director for Primary Care in 2018 and our Director of Primary Care in August 2020. Emily is also an Assistant Professor at the Yale School of Nursing.

    Ms. Coover earned her Masters of Science in Nursing in Family Practice at the Columbia University School of Nursing in 2016. She joined Janian as a Primary Care Provider in 2016 and subsequently became our first Street Medicine Provider in 2016 before becoming our first Medical Director for Street Medicine in 2018.

    Ms. Brown earned her Masters of Science in Nursing in in both the Adult-Gerontology Primary Care and Women’s Health at Vanderbilt University in 2017. She is board certified as both an AGPCNP and WHNP from the ANCC and NCC respectively. Kaitlin joined Janian as a Primary Care Provider at Janian in 2017. In 2018, she received a specialization in HIV care from AAHIVM and completed training to be a buprenorphine prescriber through the SAMSHA/DEA waiver program. Kaitlin became Nursing and Medical Assistant Supervisor in 2019.
